HOUSING AND SUPPORTIVE SERVICES

There is no limit on length of stay in our program. We provide permanent, supportive housing for homeless women living with mental illness.

Program

Cedars HOPE currently provides affordable, permanent supportive housing to 13 women with mental illness living in two intentional communities. An intentional community is a community in which un-related individuals with a common need, agree to live together in an environment of respect, cooperation and mutual support. The Cedars HOPE model of intentional communities fosters psychiatric and housing stability, self -sufficiency, and growth. Within this model, Cedars HOPE staff provides supportive services that help our residents break the cycle of psychiatric crisis and homelessness, maintain their independence and improved self esteem. We provide supportive staff that monitor and assist our residents with developing symptom and illness self- management skills. Staff is also available to help residents maintain or learn new skills necessary for successful daily living. Cedars HOPE staff provides the following:

Daily Living Skills

Staff assists residents with the challenges of daily living. They work with the residents to develop daily living skills including making personal and financial decisions, hygiene, cooking, menu planning, laundry, shopping, housekeeping and interpersonal skills.

Case Management

Staff assists residents with medical and psychiatric appointments. They attend these appointments regularly, advocate for the resident with their providers and provide feedback as needed. Staff assists with the application process for benefits and entitlements for which the resident is eligible and Cedars HOPE, Inc. will serve as Representative Payee upon request of the resident and appointment by the Social Security Administration. Communication and coordination with all treatment providers is a critical Case Management function.

Medication / Treatment Monitoring

Medication self-administration is monitored by staff. Staff encourage residents to follow all medication and treatment plans, assists them in doing so and helps the resident communicate with their provider when the they experiences difficulty and success. Residents are monitored for any increase in psychiatric or physical symptoms and providers are notified accordingly.

Community Integration

Staff encourages and supports resident’s efforts to work, attend community treatment programs, educational opportunities, or provide volunteer services to the community. The women we serve learn to live and work as a part of a smaller community and are supported in their effort to reconnect and become active members of their larger community.
